  1. Why isn't fentanyl included in standard drug panels?

Standard drug panels were developed before fentanyl became widely prevalent in the illicit drug supply. Traditional 5-panel and 10-panel tests target specific drug metabolites using antibodies designed for common substances like marijuana, cocaine, and heroin. Fentanyl and its analogs have different chemical structures requiring specific antibodies for detection. Additionally, fentanyl can be present in very small amounts while still being potent, making detection more challenging. As fentanyl prevalence has increased, testing technology has evolved to include specific fentanyl detection, but this requires additional or modified testing panels.

  1. Can fentanyl testing distinguish between prescribed fentanyl and illicit use?

The test itself cannot distinguish between prescription and illicit fentanyl - it only detects the presence of fentanyl or its metabolites. However, when Medical Review Officer (MRO) services are included, the MRO can contact the donor to verify legitimate prescriptions. If the donor has a valid prescription for fentanyl (such as a fentanyl patch for pain management), the MRO can report the result as "negative with explanation" rather than positive. This is why MRO review is particularly important when testing for substances that have both medical and illicit uses.

  1. Can fentanyl testing detect carfentanil and other fentanyl analogs?

Our advanced fentanyl testing can detect many fentanyl analogs, including carfentanil (which is approximately 100 times more potent than fentanyl itself). The initial immunoassay screening has cross-reactivity with many fentanyl analogs, and the confirmation testing using LC-MS/MS technology can specifically identify different fentanyl-related compounds. However, as new synthetic analogs are continuously developed, testing technology must constantly evolve.   1. How much fentanyl needs to be present to test positive?

The detection threshold (cutoff level) for fentanyl in urine testing is typically set at 1 ng/mL (nanogram per milliliter) for the initial screening, with confirmation cutoffs around 0.5 ng/mL. These extremely low cutoff levels are necessary because fentanyl is potent in very small amounts. For comparison, the standard cutoff for morphine (an opiate) is 2,000 ng/mL. These low detection thresholds mean that even minimal exposure to fentanyl can potentially trigger a positive result, allowing for early detection of potential fentanyl use or exposure.

  1. If someone tests positive for fentanyl, does it mean they knowingly used it?

Not necessarily. One of the most concerning aspects of fentanyl is that it's frequently mixed with other substances without the user's knowledge. Individuals using illicit drugs like heroin, cocaine, or counterfeit prescription pills may be unknowingly exposed to fentanyl. A positive fentanyl test indicates presence in the system but doesn't indicate whether the exposure was knowing or unknowing.
